Acute and Chronic Injuries of the Talar Dome

چکیده
“Osteochondral lesion of the talus” (OLT) is a broad term, representing acute and chronic disorders of the osteochondral junction. They mostly occur at the convex parts of the talar dome. At time of diagnosis signs of degenerative joint disease are rarely seen. The terminology is varied including terms like osteochondritis dissecans, ostechondral fractures, transchondral fractures. Etiology of these lesion often is unclear. Besides obvious trauma, overuse, growth disturbance and reduced vascular perfusion is discussed. Elias et al (2007) showed a medial location in 62% and lateral in 34%. It is well known that the medial lesions generally are larger and deeper than the lateral ones.

The incidence of lateral talar dome lesions in SER IV ankle fractures.
Osteochondral lesions of the talar dome are commonly the result of ankle trauma. While the technique of surgical repair of ankle fractures has been well reported, there are no studies that correlate the presence or absence of talar dome lesions. A possible explanation for this may be lack of intraoperative inspection of the talar articular surface. This retrospective study evaluates the inciden...
متن کاملOsteochondral lesions of the talus: localization and morphologic data from 424 patients using a novel anatomical grid scheme.
BACKGROUND The primary aim of this study was to evaluate the true incidence of osteochondral lesions on the talar dome by location and by morphologic characteristics on MRI. Because no universally accepted localization system for talar dome osteochondral lesions currently exists, we established a novel, nine-zone anatomical grid system on the talar dome for an accurate depiction of lesion locat...
متن کاملPantalar Dislocation without Extrusion
Open hindfoot injuries result from high-energy trauma and include soft tissue disruption, joint dislocations, with possible chondral and osseous injuries. The term “total talus dislocation”, is an uncommon open hindfoot injury associated with talar extrusion. However, a total talus dislocation without extrusion has not been reported. This case presents an open hindfoot injury with dislocations ...
متن کاملArthroscopic debridement and bone marrow stimulation for talar osteochondral lesions: current concepts
Symptomatic osteochondral lesions of the talus are a common cause of ankle pain and frequently occur among recreational and professional athletes. These lesions are being increasingly diagnosed and a great variety of treatment approaches exist. Since the talar cartilage has poor repair capacity, the aim of treatment paradigms is to restore the joint surface with a repair tissue as similar as po...
متن کاملExtra-articular extrusion of an osteochondral fragment of the talar dome.
Osteochondral fractures of the talus are uncommon. They are classified according to Berndt and Harty, as progressing in severity through four stages. This classification, however, does not address extra-articular extrusion of the osteochondral fragment. We report an osteochondral lesion of the talar dome which presented as an extruded extra-articular fragment in a closed injury of the ankle. Th...
